Position Overview:
We are hiring three enthusiastic and compassionate nurses to join our growing Remote Patient Monitoring (RPM) team
Key Responsibilities:
Clinical RPM Operations (70%)
- Assist in enrolling eligible patients into the RPM program.
- Monitor and document patient vitals such as blood pressure, glucose, and weight through digital platforms.
- Perform monthly check-ins via phone to support care coordination and education (aligned with CPT 99457/99458).
- Train patients and families on the use of devices and remote monitoring tools.
- Flag critical readings and escalate them to supervising nurses or physicians.
- Ensure accurate documentation and HIPAA compliance.
Sales Presentation & Provider Support (30%)
- Support the sales team by attending physician meetings (in-person or virtual) to explain the clinical value of RPM.
- Conduct basic walkthroughs of how RPM works for patients and providers.
- Serve as a clinical voice to help answer questions from new clinics and practices.
- Collect provider feedback and help identify areas for improvement or new service opportunities.
Who Should Apply?
- New graduates (LVN or RN) looking for a digital health opportunity.
- Nurses with 1–3 years of clinical experience interested in remote care and patient engagement.
- Individuals with good communication skills and a willingness to learn sales/customer-facing skills.
Minimum Qualifications:
- Active RN or LVN license (in good standing).
- Strong verbal and written communication skills.
- Comfortable using technology (RPM platforms, apps, video calls, etc.).
- Compassionate and proactive approach to patient support.
Preferred (Not Required):
- Experience in telehealth, home health, care coordination, or primary care settings.
- Familiarity with Medicare RPM billing codes: 99453, 99454, 99457, 99458.
- Exposure to EHRs and digital health tools.
- Bilingual (English/Spanish) is a plus.
